A little about working in our team
Working in this team is all about the helping people to leave hospital and safely settle back into independent living.
You would work with people to identify their ongoing care and support needs. Helping them to recover in a more relaxed environment.
We work with a range of health and social care professionals like Nurses, Physiotherapists, Occupational Therapists, Social Care Workers or GPs.
Our aim is to:
- encourage people to regain confidence in their own ability to manage
- provide an environment that improves someone’s general physical and emotional wellbeing, whilst enabling them to make important decisions about their future
- reduce the risk of that person having to return to hospital
Types of job roles in our team
Team Manager
You would work with colleagues and partners within the hospital multidisciplinary team to plan and co-ordinate the health, welfare and social care support through overseeing the work of other staff.
Social Worker
You’d support the safe hospital discharge of vulnerable people by assessing their support needs. You’d work alongside a multi agency team. Together you will make recommendations to ensure that appropriate care and support is provided to people and carers.
Community Care Officer
You’d support community engagement activity – specifically with voluntary and community organisations. This is part of the hospital discharge process with the aim of aiding a person’s rehabilitation and recovery.
Access and Information Advisor
You would provide advice and information to a person and where appropriate their carers. You’d signpost them to support services and formal assessments.
