A little about working in our team

Working in this team is all about helping older people and those with physical disabilities, to stay healthy, maintain independence and have choice and control over their lives.

We offer a flexible and personal approach that puts the individual at the heart of everything. We help to develop a personal support plan where each component makes an important contribution to the individual’s care.

Our aim is to:

  • be closely involved in developing a care plan that is right for people with our help and that of carers, GPs and district nurses
  • offer an indicative budget for this care plan to help design their individual support
  • implement, monitor and review the support plan to ensure the personal outcomes have been achieved

Types of job roles in our team

Team Manager

You’d would plan and coordinate the health, welfare and social care support through overseeing the work of the team.

Social Worker

You’d help vulnerable people by assessing their support needs. We use something called ‘a strength-based approach’. You’d make recommendations which increase independence and where necessary complete assessments for care packages.

Community Care Officer

You would work alongside Social Workers to review current packages of care, supporting and enabling service users to maximise their independence. You’d always take into account service user choice and carer concerns.

Community Outreach Support Worker

You would support community engagement activity – specifically with voluntary and community organisations that provide support to vulnerable groups as part of our wider prevention and early intervention support solutions.

Transition Managers

You’d help provide information, advice and guidance to managers with regards to the requirements, legislation and outcome measures. Also, you’d help develop and review departmental policies, specifically focussing on policies that relate to Operational Services. There are some complex issues/problems to deal with on a regular basis, so using your own initiative to reach creative solutions is required.
